Add OpenCode setup docs

Adds a dedicated OpenCode section to README covering:

• rtk init -g --opencode
• plugin install location (~/.config/opencode/plugins/rtk.ts)
• how the tool.execute.before rewrite works
• restart / verification flow

Also removes the earlier proposal-only file so the PR contains the actual docs update.
