Can't add a callback url

My app (personal access) has been approved. But can't complete because there is no input in this page https://www.etsy.com/developers/edit/1234/callbacks there is a button "Add callback URL" has no function on click. and 2 other buttons cancel (this has no function too) and save (works but there is nothing to save)
