test: single-gate triage smoke test

Smoke test for the single-approval-gate change in #748.

Expected behavior:

• pre_activation runs immediately (no prompt)
• activation runs immediately (no prompt)
• agent queues, single approval prompt appears
• After approving once, agent runs and downstream jobs (detection/safe_outputs/conclusion) run without further prompts
• Triage comment + label appear on this issue ~2-5 min after the single approval

Sample content for the agent: this is another meta smoke test to validate the dual-environment split (gated agentic-triage on the agent job, ungated agentic-triage-passthrough on the rest). Apply a wontfix label and note this is a test.
